How do I book?

Appointments can be scheduled by calling our office directly at 903-350-9559. Once you are an established patient, we offer convenient online scheduling through our patient portal linked on our home page.

What forms are needed?

Please complete all required forms before your visit; they’re available through the patient portal.

Which insurances do you accept?

We accept most major insurance plans: Blue Cross Blue Shield, Aetna, Cigna, United Healthcare, and Medicaid (coming soon). Contact us if you have questions about your specific provider.

What are your hours?

Our office is open Monday through Friday, 8 AM to 5 PM, closed 12pm-1pm for lunch.

What is your cancellation policy?

Please notify us at least 24 hours in advance to avoid cancellation fees.
